I understand that I am responsible for my own behavior as well as the conduct of my team as a whole. I vow to play with fairness and integrity with regard to the game, officials, my team and my opponents. I am aware that in the event I do not uphold this pledge, I and/or my team may be disqualified from the tournament. I also certify that the information is correct and accurate to the best of my knowledge and have obtained permission from all parents or guardians where necessary.

I know that participating in strenuous activities such as basketball is potentially hazardous. I assume all risks associated with playing in this event including, but not limited to, falls, contact with other participants, effects of the weather, and the condition of the playing surface, all facts being known and appreciated by me. Knowing these facts, and in consideration of you accepting my entry, I hereby for myself, my heirs, executors, administrators or anyone else who might bring claims on my behalf, covenant not to sue, and waive, release, and discharge Hibachi Events LLC, all charities benefiting from this event, Hibachi Events LLC employees, volunteers, and any sponsors, including their agents, employees, assignees, or anyone acting on their behalf, from any and all claims of liability for death, personal injury, or property damage of any kind or nature whatsoever arising out of or in the course of participating in this event. This Release and Waiver extends to all claims of every kind or nature whatsoever, foreseen or unforeseen, known or unknown. I hereby grant full permission to any and all foregoing to use photographs, videotapes, motion pictures, recordings, or recordings of me or my likeness from this event for any purpose. Applications of minors will be accepted only with a parent or guardian’s signature and must also be signed by the minor.
